Breaking Free from the Trap: Understanding and Halting The Sexual Pain Cycle

If you are one of the many women who experiences pain during sex (dyspareunia), when using tampons, or during a routine gynaecological exam, please know this: You are not alone. This condition, often termed Chronic Pelvic Pain or Sexual Pain Syndrome, is incredibly common, yet often silently endured.
As a Pelvic Floor Specialist in Beckenham, my mission is to shed light on this experience and, more importantly, to help you find lasting relief. The pain you feel is real, but it doesn't have to be permanent.
Let’s take a closer look at the concept I call The Sexual Pain Cycle—a distressing loop that traps the body and mind, and how Women's Health Physiotherapy is the key to breaking it.
🔁 The Sexual Pain Cycle: A Self-Sustaining Loop
The image above illustrates this cycle clearly. It shows how initial physical pain can lead to psychological changes and behavioural patterns that, unintentionally, make the pain worse and keep the cycle on repeat.
1️⃣ Initial Pain Trigger (THE START)
The Reaction: “It just HURTS when I try.”
The cycle begins with a physical event—whether it’s vulvar pain, a change in hormones causing vaginal dryness/tissue thinning, or an unidentified underlying cause—leading to an experience of pain or discomfort.
2️⃣ Muscle Spasm & Tension (PHYSICAL REACTION)
The Reaction: “My pelvic floor instantly TENSES UP.”
This pain triggers a protective mechanism in your body. The muscles of the pelvic floor, often without conscious command, go into spasm or become hypertonic (too tight). This tension is the body’s way of 'guarding,' but it is precisely this muscle tightness that causes the chronic pain loop.
3️⃣ Brain Remembers Pain (THE MEMORY)
The Reaction: “My brain expects pain. Hello, ANXIETY.”
The brain creates a strong association between the activity and the pain (a concept called central sensitisation). Even thinking about intercourse or a gynaecological exam can trigger stress or anticipatory pelvic tension.
4️⃣ Guarding & Avoidance (THE COPING MECHANISM)
The Reaction: “I avoid sex/tampons/exams completely.”
To cope, you naturally start to avoid the triggers. This avoidance, while understandable, reinforces the brain's association of the area with danger and deepens the cycle. This is why getting specialized help from a Beckenham Women's Health Physio is crucial.
5️⃣ Chronic Pain Cycle (THE TRAP)
The Result: “The pain cycle is now on REPEAT.”
The tension, the anxiety, and the avoidance all feed back into the initial trigger, reinforcing the chronic pain state, such as conditions like vaginismus or vulvodynia.
